At 48 minutes and 5 seconds, their match became the longest match in the January 4 Tokyo Dome Show history. After the match, Ibushi was challenged to a title match by SANADA. Following the event, Ibushi expressed his ambitions of unifying the IWGP Heavyweight and IWGP Intercontinental Championships.

Inaugural IWGP World Heavyweight Champion, pursuits and departure (2021–2023)[]

On February 11, at The New Beginning in Hiroshima, Ibushi successfully defended IWGP Heavyweight and IWGP Intercontinental Championships against SANADA. In a attempt to prevent his ambition, Tetsuya Naito which Naito deemed unacceptable, he challenged Ibushi to a match for the IWGP Intercontinental Championship. This led to on February 28 at Castle Attack, Ibushi successfully defending the title against Naito. Two days later, during a press conference, NJPW Chairman Naoki Sugabayashi announced that both titles would be unified into the IWGP World Heavyweight Championship under a new lineage, while carrying both championships legacies, with Kota Ibushi being recognized as the inaugural champion. However at Ibushi's request, NJPW announced that Ibushi would make his last defense of the IWGP Heavyweight and IWGP Intercontinental Championships against reigning IWGP Junior Heavyweight Champion El Desperado, who had challenged him to a title match at Castle Attack, on March 4 at NJPW's 49th anniversary event, with the winner of the match being crowned the first champion. At the event, a new IWGP World Heavyweight Championship replaced the IWGP Heavyweight and IWGP Intercontinental Championships and Ibushi defeated Desperado to become the inaugural champion. He lost the title to 2021 New Japan Cup winner Will Ospreay on April 4 at Sakura Genisis.

On June 7, at Dominion 6.6 in Osaka-jo Hall, after Shingo Takagi won the vacant IWGP World Heavyweight Championship, Ibushi was nominated by Takagi to be his first challenger for the title, which Ibushi promptly accepted by him. On July 11, it was announced that Ibushi would be forced to miss an event during the Summer Struggle tour, after suffering from side effects from the COVID-19 vaccination. This effects also led to Ibushi missing most of the tour. After making his in-ring return on July 19, it was announced on July 21, that Ibushi was diagnosed with an aspiration pneumonia, which forced him to miss several NJPW events, including the main event of Wrestle Grand Slam in Tokyo Dome on July 25, which in turn led Ibushi to be replaced by Hiroshi Tanahashi. On August 14 at Resurgence, after Tanahashi won the IWGP United States Heavyweight Championship, in a post-match interview, Tanahashi nominated Ibushi to be his challenger. Four days later, Ibushi, who had been recovering from an aspiration pneumonia, accepted Tanahashi's challenge. This led to on September 4 at Wrestle Grand Slam in MetLife Dome, Ibushi unsucessfully challenging Tanahashi for the IWGP United States Heavyweight Championship. From September 18 to October 18, Ibushi took part in the 2021 G1 Climax, where he won his block with a record of seven wins and two losses, besting both Hiroshi Tanahashi and reigning IWGP World Heavyweight Champion Shingo Takagi, advancing to the finals. On October 22, Ibushi was defeated in the finals by Kazuchika Okada via stoppage, after Ibushi dislocated his right shoulder. In February 2022, Ibushi was expected to return to in-ring competition to face Great-O-Khan in the first round of the 2022 New Japan Cup. However, Ibushi was forced to forfeit his match, after being medically advised to not take part in the tournament, as part of his recovery from the shoulder injury.

In May, Ibushi began expressing his frustration with NJPW management on social media. The issues stemmed from how Ibushi was treated by a member of NJPW talent relations, along with Ibushi feeling that NJPW was trying to rush him back from his shoulder injury too soon, while claiming that various wrestlers working for NJPW were being exploited. The talent relations official, most notably, Head of Talent Relations Yusuke Kikuchi, who threatened to terminate Ibushi's contract for making an appearance to accompany wrestlers to the ring at Taka Michinoku's Just Tap Out promotion without NJPW's permisson to appear at the event. Ibushi openly stated that he did not agree with the unification of the IWGP Heavyweight and Intercontinental Championships. Ibushi also alleged that the upper managment in NJPW have been involved in sexual harassment, as well as dealing with the Yakuza. Ibushi was also upset with Kikuchi for allegedly withholding merchandise money that owed to him back in 2018 from the Golden Elite t-shirts and blocking a planned match with Shingo Takagi. Ibushi ceased communications with the NJPW office, after being controversially removed from the 2022 New Japan Cup by Kikuchi on the advice of a doctor. NJPW booker Gedo acted as a peacemaker between both sides. Later that month, Ibushi noted that Kikuchi and NJPW President Takami Obari refused to apologize or meet with him and with the stress of the situation causing his mother to attempt to commit suicide, by suffering an injury in her attempt. In the aftermath of this situation, various reports indicated that Bushiroad stock price had fallen down as a result of Ibushi's public criticism, with the upper managment of the company not being satisfied with the bad press around the situation.

On May 27, during a press conference, NJPW publicly address the matter, apologizing to Ibushi, while also announcing a wage cut to NJPW Representative Director Takami Obari and to the referred official. Obari revealed he met with Ibushi on March 31, where the two briefly trained, and claimed that Ibushi had told him his recovery time was taking longer than expected. Following a conversation between both sides, Obari revealed he reprimanded Ibushi for his actions, but accepted Ibushi's apology and found that Ibushi was not attempting to breach his contract, with both Ibushi and Kikuchi being punished with 10% salary cut for one month and Kikuchi being transfered from his talent relations role to another role within the promotion before being released later that year as a result of the situation.

Shortly after, Ibushi revealed that various details about their meeting were purposely not revealed, while also believing that the situation caused a negative reaction, with various fans turning on him for revealing negative things about NJPW. In July, it was reported that the issues with Ibushi and NJPW had calmed down, although Ibushi's actions were not fully settled. The following month, the Wrestling Observer Newsletter reported that Ibushi was not looking to return to the promotion, with people within beliving he was not looking to return to the promotion going foward. On December 2, it was revealed that Ibushi's contract with NJPW was scheduled to expire at the end of January 2023, with Ibushi awaiting for his contract to expire. On January 31, 2023, his profile was removed from NJPW's website, signalling his departure from the promotion once again.

Pro Wrestling NOAH (2024)[]

On November 2, 2023 at Noah the Best, Ibushi made his return to Pro Wrestling Noah after eleven years, confronting Naomichi Marufuji after his match, challenging him to a match at NOAH The New Year. Marufuji accepted, stating that he was "waiting for him in the ring at Ariake Arena", before both shaking hands, with the match being billed as "Destiny". Afterwards, NOAH announced that the match would be the main event of NOAH The New Year. Afterwards, the annoucement for the main event mixed reactions among fans, including criticism, mainly on social media, leading NOAH's Executive Officer Narihiro Takeda to reveal that he attributed them the main event, due to the popularity of both wrestlers over the GHC Heavyweight Championship match. At the event, on January 2, 2024, Ibushi faced Marufuji. During the match, Ibushi reportedly suffered injuries to both of his ankles, however, he managed to win the match. Afterwards, Ibushi was taken to a hospital by an ambulance. Shortly after, NOAH's Executive Officer appeared backstage to announce that Ibushi would not be making any post-match comments.
